Please download and complete the Adoption Application and send it to our office, along with a personal check
or Money Order in the amount of $250, made payable to Sunshine International Adoption Agency, as well as
the following 4x 6 photos: two different, non-professional, photographs of adoptive couple together, one
photograph of the outside of the home, one photograph of other children in the home and three interior
photographs of the home.  Please write your family’s name on the back of all photographs.  

You must fill in the form completely for Sunshine International Adoption Agency to process your international
adoption application.
